Lifestyle Education and Exercise Program (LEEP)

The Lifestyle Education and Exercise Program (LEEP) promotes overall wellness in patients with confirmed cerebral vascular disease.  The program’s mandate is to empower patients with the knowledge to make the best decisions for themselves in creating a healthier lifestyle.  

Individuals with confirmed cerebral vascular disease including those who have had a stroke or transient ischemic attack (TIA) can benefit from this service.

Individuals with the following risk factors may also benefit from this program:

  • High cholesterol
  • High blood pressure
  • Obesity
  • Smokers
  • Diabetes
  • Increased stress levels
  • Inactive lifestyles

The following health benefits are goals for clients who actively participate in the program:

  • Improvement in cardiovascular fitness
  • Weight loss
  • Overall increased feeling of well being
  • Improved blood pressure
  • Improved blood sugar control
  • Improved cholesterol and triglyceride levels

Care We Provide

As part of the six month treatment plan, patients are encouraged to participate in the following:

Orientation Sessions
An introduction to the services provided by the Physiotherapist and other members of the team.

Introductory Assessment
The Introductory assessment is conducted to evaluate risk factors and create a personal care plan to assist in making client’s lifestyle behavior changes.

Educational Sessions
Each week the program offers education sessions on topics such as stress management, healthy eating, and medication management and more.

Exercise Sessions
Following completion of a personalized exercise prescription by the program’s physiotherapist, participants will attend weekly education and exercise sessions over a six month period.  Following graduation from the program we will connect clients with community partner experts in long-term lifestyle choices to best manage cerebrovascular disease as well as ongoing exercise venues.

Stress Testing
Physician supervised, graded exercise tests are conducted at regular intervals when participating in the program and are important tools for evaluating each client’s progress.

  • Physician Consultant
  • Diagnostic Team
